If you or your family members have any of the following please notify our staff immediately:
- Recent international travel, a cruise vacation, and/or air travel in the United States.
- Had possible contact with individuals with COVID-19 virus infection in the United States
- Experiencing symptoms of fever, cough, shortness of breath
Please do not enter the office without talking with the office staff. Call our office 904-829-6591 immediately.
We are offering Telehealth services for non-emergent follow-up appointments that can be evaluated through a video call. Please call our office to see how to set up this Telehealth visit.

Appointment Cancellation Policy
We DO NOT take walk in appointments. All referrals must be given by your Primary Care Physician. In an effort to have as many appointments available to our patients as possible, it is important to give our staff 24 hours’ notice if you need to cancel or reschedule an appointment. IDANF PA may charge you a no-show fee of $25 if the 24-hour notice is not provided. Thank you for your cooperation in this matter.
